It’s amazing to me that people are continually amazed that “…a hospital contributed to the deaths of five children”
This happens all the time (over 1000+ times per day). It is no secret. Here is but one study
A study in the Sept. 2013 issue of the ‘Journal of Patient Safety’ said that between 210,000 and 440,000 patients each year who go to the hospital for care suffer some type of preventable harm that contributes to their death.
