I’m editor of an academic journal; we provide a LaTeX document class file to authors (which is not the final class file we use in publication), it is remarkable the extent to which authors will go to to circumvent the restrictions we try to impose. In some cases this does include using subtle font variants (eg, the lmodern package) which increase the spacing.

Sometimes it is the only part of a paper not behind a paywall.

Yes, sadly. More often than you would think given that the students are all adults (though less often than my High School teacher friends have to deal with it - yeesh).
