An identical mistake resulted in the death of a woman in a tandem hang-gliding accident east of Vancouver a few years back. Tragically, she wasn’t able to hang on.
Gotta wonder what waiver the passenger signed or what insurance the instructor carried.

I dunno if it’s still true, but years ago the main way hang-gliding pilots used to die was from forgetting to clip themselves on… only takes that one time.
