For me, this looks like the only tool that would allow me to achieve Inbox Zero.
Postfix, qmail or exim and procmail on a very minimal Linux server in the real internet (DNS, SPF and MX records required) can do that out of the box. Insult database not included. Might require help from somebody experienced running and setting up email servers.
