As long as it’s in international airspace, you’re covered by the medical Law of the Briny Deep, so anything goes, plus you get to keep anything you find in there by right of salvage.

I wonder what would happen if one starts a procedure over intl’ waters but partway during the procedure the airliner flies into a country’s jurisdiction.
It’s people like these that remind you what a real doctor can do when they improvise.
