I use a small random orbit sander with a non-woven abrasive pad:

The specially ordered pads are only 3" diameter but they still fit the 3 1/8" sander:

This is a lot more effective on old NYC tubs than a rotary scrub brush, but still doesn’t scratch the enamel.

You could also buy shank-mounted abrasive pads for a power drill but suffer a tenth of the effectiveness since you can’t push down hard without losing control of it.
