The drawing prompted a teacher to take the photo and have a meeting with a social worker.

It’s a cute pig. That poor kid. At most this warranted a heads-up to the parents or low-key discussion with the artist about how images can be misconstrued.
And a talk with the boy tattletale, perhaps with a basic anatomy lesson. At that age it’s normal for a boy* to see penises everywhere but he needs to learn to shut his trap about it.
Maybe also normal for girls. But it was always the boys pointing out every penis-adjacent shape and giggling about it in that strange giggle they use when they are trying to sound mature
