Employees don’t have “health permits”… the business gets checked by local health inspectors and are graded, not the people who work at the restaurant…

In my state, in order to work in food service, you must get a permit from the health dept. You have to take a training class and pass a test. They used to screen you for hepatitis also, but apparently that’s no longer done.
[ETA] I’m sure this is remnant from when Texas was still blue.

Really? Not in mine… at least not when I worked in food service. I did have to get a permit to serve alcohol, but that’s it. TIL, I guess. So, your state might, but not all states do.
Probably a racist byproduct, of keeping out POC, actually. Much like today, there is a focus on immigrants and any POC as being vectors for carrying diseases.
