Let’s not forget that Tennessee’s governor and legislature continue to ignore a real crisis in the Department of Children’s Services because they’re more interested in attacking LGBTQ people.
And with their silence they offer support for violence.

The dilemma is more acute if the person seeking care is a child or adolescent. This is the complex and difficult challenge that specialists in gender dysphoria must master to provide the best possible care to young people. John Launer describes the hostility and criticism that colleagues experienced at London’s Tavistock Clinic in striving “to make the best decisions they could in a situation where evidence was thin and the politics noisy” (doi:10.1136/bmj.p477).1

I am going to disagree with this part of the statement. It is almost never “experts” who deal with these issues on the front end, it is the PCPs who are hassled, harried and overscheduled. I have had to make myself familiar with the ins and outs of embryology as it affects sexual development, psychology and politics of trans as well as anti-trans folks, and resources available for this population in a not-too-terribly trans friendly area. I would point out that this is an area that was not even brought up when I was in training other than a brief mention when talking about psychiatric disorders. By the time most of my patients get to a specialist, they have already had to navigate a significant maze of social and medical hurdles. As complex as the author portrays the situation, the reality is much more so.

. An investigation by The BMJ finds that more and more young people are being offered medical and surgical intervention for gender transition, sometimes bypassing any psychological support (doi:10.1136/bmj.p382).5

This also sticks out to me. Psychological support is always the first thing we do. Medical transitioning and, even more so, surgical transitioning, without psychological support is verging on malpractice.
