An ex-prison official with cancer. Given his supposed familiarity with how illnesses are handled in Russian prisons, he may have felt a great need to expedite things and in as painless fashion as possible.
That was my thought. Not only would he know what conditions were like but it it just possible some of the other prisoners might take a bit of revenge.
