Honestly, he looks normal for a man of his age and diet and lack of exercise.
It’s just his vanity with makeup and hair placement and plastic surgery that makes him ashamed to be seen like this.
That actually might be the nicest photo I’ve ever seen of him. He’s not doing anything with his face besides looking at something. Everything else he is yelling or sulking or smirking or otherwise expressing contempt. Shame about the fascist hat though.
