It was a good first step, Obamacare (based on Romneycare) toward rescuing a sector of our economy from its own greed. The time is right to take a second step, which is offering single payer (medicare for all) as an option.

I’d take that option in a heartbeat.

The ACA, being in its bones a republican healthcare plan, is big on ‘skin in the game’, the idea that the reason healthcare costs are high is that the end users are protected from paying for them directly, and that this will therefore be solved by having high deductibles, high copays, etc.

So many of its plans has high deductibles. Bronze plans in particular are a form of ‘catastrophic’ insurance, that pays out little until your bills for the year are over some thousands of dollars, often 5-6 thousand. Silver plans still have high deductibles. However the ACA also covers some things before the deductible.

There is an element of truth to it. The ACA largely fixed the way individual plans were starting to not cover you if you did get really sick, but it still leaves the potential of paying a lot out of pocket, possibly year after year.

So, the solution to this? Lower the deductibles, etc. This means higher premiums. That means more subsidies.

The ‘skin in the game’ idea is stupid. Every other wealthy country does far less of that and has healthcare costs far below us.

Every other part of our healthcare system has less ‘skin in the game’ than individual insurance, and they all work better.

Pitting sick and suffering individuals against expert doctors and corporate healthcare giants with the idea that they will negotiate lower costs and better outcomes is based entirely on fantasy.
